Permanent Jewelry Liability Waiver & Client Agreement

Acknowledgment & Consent

I voluntarily choose to receive permanent jewelry services from Angel Jess 444. I understand that permanent jewelry is custom-fit and welded closed without a clasp.

I acknowledge that:

   -The welding process creates a small spark during application. Safety glasses will be provided and must be worn during the welding process.

   - I have informed the artist of any medical conditions, allergies, skin sensitivities, or concerns that may affect this service.

   -I understand there is a small risk of skin irritation, burns, allergic reactions, or discomfort associated with the application of permanent jewelry.

  - I understand that permanent jewelry is intended to be worn continuously but may need to be removed for medical procedures, certain occupations, travel, sports, or other personal reasons.

Jewelry Removal

If I need to remove my jewelry in an emergency or for any reason, I understand I should cut only the jump ring using wire cutters. This preserves the chain so it can be re-welded later.

Repair Policy

Angel Jess 444 guarantees the weld for 7 days from the date of application.

- If the weld fails within 7 days due to normal wear, it will be repaired at no charge.

- After 7 days, rewelding is available for a $25 welding fee.

- This warranty does not cover lost jewelry, stretched chains, damaged chains, improper removal, misuse, accidental damage, or normal wear and tear.

Refund Policy

All permanent jewelry sales are final.

Due to the custom nature of permanent jewelry, no refunds, exchanges, or returns will be provided once the jewelry has been welded.

Release of Liability

I understand that by receiving permanent jewelry services, I voluntarily assume all risks associated with the service.

I release and hold harmless Angel Jess 444, Jessica Turner, and any employees or representatives from any claims, injuries, damages, losses, or liabilities arising from or relating to:

- The application or removal of permanent jewelry

- Allergic reactions or skin sensitivities

- Improper care or maintenance

- Normal wear and tear

- Loss or damage of jewelry after my appointment

I understand that results may vary and that no guarantees are made regarding how long my jewelry will remain intact.

Minor Consent

If I am signing for a minor, I certify that I am the minor's parent or legal guardian and authorize the minor to receive permanent jewelry services. I accept full responsibility for the minor and agree to all terms of this agreement on their behalf.

Photo & Video Consent (Optional)

At the time of your appointment, Angel Jess 444 may ask for your permission to take before and after photos and/or videos of your permanent jewelry. Your consent is completely voluntary, and declining will not affect the services you receive.

If you choose to allow photos or videos to be taken, they may be used on the Angel Jess 444 website, social media pages, marketing materials, and other advertising platforms.

 I understand that I may be asked at the time of my appointment for permission to take before and after photos and/or videos, and I consent to their use for promotional and marketing purposes.

Agreement

By signing below, I certify that:

 I have read and understand this entire Liability Waiver and Client Agreement.

- I have had the opportunity to ask questions.

- I understand the risks involved.

- I voluntarily consent to receive permanent jewelry services.

- I agree to all policies outlined above.
